Onboarding Questionnaire

Please answer all questions as honestly as possible, there is no judgement, no wrong answers and everything will be confidential. The more information that you provide the more detail I will be able to provide when writing your plan and advising in check-ins.

Where there is a rating from 1-5. 1 is the worst/ lowest and 5 is the best/ highest.

Our Promise

Walking into a new environment can sometimes be a little bit scary however my promise to you is that I will provide a supportive environment, encouraging you in your progress.

We want to see change and I know that you’re willing to work for it as such, ask all the questions that you need, turn up on time, have fun and enjoy.

I’m not here to judge you. I’m here to help you be the best version of yourself that you can be.

Let’s get straight to it!

We’ll start our fitness journey from a different place, which is absolutely fine.

To help make sure that you’re fueling your body right for exercise it would be great if you could fill out a food diary for the next few days, being honest with both yourself and with me about what it is that you’re eating and drinking. There is no judgement on this, it’s just our starting place to look at the changes we might need to make in order to help us live a healthy lifestyle.

Within everyday life, we do a lot of movement be it sitting down standing up reaching for something, as a result here are two key exercises I would like you to practice before we meet in person for our first session
